Even if you have just a couple stages of health. If you have visual cues to things like inebriation or poison that are different from the various stages of health, that would also give a cue to other players. This would necessarily be used against them. Perhaps if your character sells medicine, you could find a person that looks especially sick. By using medicine that you normally sell, you might be able to gradually heal the other player. There are so many different ways to indicate injury without actually showing health bar, the last one posted is a really viable idea. If the player is fighting, holding the sword with his two hands, how can you see the cue? You wouldn't stop fighting just to hold your stomach, letting your enemy know that you're already done for. If you dont see any cue, may be unrealistic; because in combat you can have the option to dont kill the enemy (useful for roleplaying); or you know when you must heal (or protect) your partner; or to prevent the stupid player that provoke you when he's at 1 hit to die. He could start stumbling or swaying during battle, or he could even take his right hand off his sword (the guiding hand) to hold his stomach when he's not blocking or attacking.
